We are looking for a compassionate, organized, and dependable Case Coordinator to join our team. In this role, you will serve as an important liaison between clients and the people who support them, including family members, government workers, caregivers, and medical professionals.
The Case Coordinator plays a key role in helping individuals work toward their personal goals while ensuring their support team stays informed, organized, and connected.
Key Responsibilities
- Serve as the primary point of communication between clients, families, government workers, caregivers, medical professionals, and other members of the support team.
- Build positive, trusting relationships with clients to better understand and support their individual needs and goals.
- Conduct regular visits with assigned clients to check in on their well-being, progress, and services.
- Support individuals in identifying and working toward their personal goals.
- Coordinate communication and services among members of each client's support team.
- Document client interactions, visits, progress, concerns, and other important information accurately and in a timely manner.
- Oversee and communicate with caregivers assigned to various cases to help ensure clients are receiving appropriate and consistent support.
- Identify concerns or changes in a client's needs and communicate them to the appropriate members of the support team.
- Help ensure services and care plans are being followed and that clients' needs are being addressed.
- Maintain confidentiality and professionalism when working with sensitive client information.
- Complete other case coordination and administrative responsibilities as needed.
